Delta Wellbeing is a Local Authority Trading Company, owned by Carmarthenshire County Council, which provides assistive technology and proactive monitoring to support older and vulnerable people to live independently.
Since the company was set up in 2018, we have expanded our services to provide a range of innovative solutions in the health and social care sector, transforming the way social care is being delivered across west Wales by implementing a new model of self-help and pro-active care.
We are extremely passionate about what we do, delivering a single point of access for health and social care in Carmarthenshire, and supporting patients leaving acute settings as well as in the community, with our hospital-based ‘Blue Army’ and our 24/7 response service, the first of its kind to be registered with Care Inspectorate Wales.
Early intervention and prevention and the use of digital technology is the main theme of this year’s conference, and it could not be more relevant. Our partnership with CGI will help us to provide smarter, personalised care and support, developing a whole system approach to the continuum of care, and making sure we are at the cutting-edge of what is capable both now and in the future.”
